External medicines optimisation team to improve value, quality and safety in prescribing from Calderdale GP Practices. The service comprises pharmacy professionals who deliver an annual medicines optimisation action plan in our practices. This includes cost-effective changes to prescribing to deliver savings or cost avoidance from the primary care medical prescribing budget. The annual plan includes quality and safety in prescribing actions for Calderdale patients, encompassing reviews of specific quality in prescribing concerns and a quarterly audit programme. The team are a resource for practices to support national safety alerts and medicines shortages.

The Contract Authority; NHS West Yorkshire Integrated Care Board intends to award a contract to an existing provider following Direct Award Process C. The authority is publishing this notice in Find a Tender in accordance with the NHS Provider Selection Regime.

External medicines optimisation team to improve value, quality and safety in prescribing from Calderdale GP Practices. The service comprises pharmacy professionals who deliver an annual medicines optimisation action plan in our practices.
