Wiltshire Council is seeking a single provider to deliver an outreach NHS Health Check programme in Wiltshire.
The NHS Health Check programme aims to improve the health and wellbeing of adults aged 40-74 years through the promotion and early awareness, assessment and management of the major risk factors of Cardiovascular Disease.
The outreach NHS Health Check programme will seek to engage population groups/communities across Wiltshire
were uptake is low, or where their GP Practice is not providing the service. They will deliver NHS Health Checks to population groups/communities as identified by the local authority.
The contract period will be for an initial three years with an extension option of up to an additional two years. The service will commence on 01 April 2024.
The provision of NHS Health Checks will follow Best Practice Guidelines for this service.
