The Temporary Care Home Beds for Hospital Discharge service is an integrated person-centred approach to the safe and timely transfer of medically optimised patients from an acute hospital to a community setting. This is to enable the assessment of their long-term health and social care needs or continued recovery before being able to go home.

The service is aimed at people who no longer have a reason to reside in hospital and therefore do not require an acute hospital bed, but who still require ongoing care for a period of assessment or recovery.

As part of a whole system approach the service will offer recovery, and assessment within a care home setting.

The Temporary Care Home Beds for Hospital Discharge service is aimed primarily at older people as the single biggest group of hospital bed users nationally. With a predicted growth in the over 65 age group these pressures are likely to increase. This specification outlines a temporary measure to alleviate pressures for this group across the wider health and social care system which may be further exacerbated by the COVID-19 outbreak.

Care will be delivered to service users that have been identified by the commissioner (or organisation acting on behalf of the commissioner) as requiring up to 4 weeks rest or recuperation (with or without therapy needs) for their long term needs to be assessed which is likely to be to return home, with or without a package of care.

The purpose of Temporary Care Home Beds for Hospital Discharge service is an integrated person-centred approach to the safe and timely transfer of medically optimised patients from an acute hospital to a community setting. This is to enable the assessment of their long-term health and social care needs or continued recovery before being able to go home.

The service is aimed at people who no longer have a reason to reside in hospital and therefore do not require an acute hospital bed, but who still require ongoing care for a period of assessment or recovery.

To be able to accept referrals same day and offer an admission date that is within 24 hours of the referral being received the provider should ensure;

• Staffing levels are adequate to accept all referrals safely

• Secure email used for referrals is constantly monitored

• There is always a member of staff available to confirm acceptance and admission date

Providers are expected to accept referrals;

• Same day (where referrals are received after 4pm the response is expected by 10am the following morning) with an admission date within 24 hours of referral being received

• Where the bed type is included in the block payment i.e. dementia / nursing

• Where the assistance of two care assistants is required for transfers

• Where 1:1 is required, although where the provider provides multiple temporary care home beds for hospital discharge it is not expected that all available temporary beds will be filled with patients requiring 1:1 (see section 6.9)

• For patients who have not received the covid vaccination whether in part or in full

It would be expected that a provider would accept a patient that has been exposed to COVID-19 during their hospital stay but have negative PCR test. Providers will not be requested to admit positive covid-19 referrals.

It is recognised that providers will have other referrals to manage alongside the beds within this specification. Therefore, it will not be expected of any care home to accept more than three admissions, under this specification, in a 7-day period.

Providers are expected to fully comply with the latest government guidance regarding admission to a care home and visiting.

The provider will ensure that appropriate staffing and processes are in place to safely receive new admissions 7 days a week, between the hours of 09:00 and 18:00.

Scope of our Requirements/Specification

The CCG is looking to procure up to a total of 45 beds, initially, (up to 20 general nursing beds, up to 10 dementia nursing beds, up to 10 dementia residential beds and up to 5 general residential beds) for the purpose of providing a temporary bed for someone discharged from hospital. Providers are invited to bid for a maximum of 6 beds per care home setting. The bid can include a mixture of dementia nursing, general nursing, dementia residential and general residential as long as it doesn’t contravene CQC registration.

Participants must be able to provide beds in a care home setting within the city of Leeds.

Referrals will be sent from the bed bureau to the provider’s designated secure email address and will be allocated based on bed type available for the person’s need i.e. general / dementia / residential / nursing. Where possible geographical location based on closest distance to the patient’s home address will be used for allocation. The home will be emailed an eDID (electronic Discharge Initiation Document) that has been completed by the ward. There will be no full social work assessment prior to admission to the care home since the main purpose of the beds is to allow full assessments to take place outside of an acute setting. The decision as to whether it is an appropriate referral and confirm admission will be based on the individual’s complete and robust eDID.
