two.2.4) Description of the procurement
Reading Borough Council’s ("RBC" or the "Council") vision is to enhance the reablement approach to arranging, delivering, and reviewing care that supports Individual’s (otherwise coined as ‘service users’) to do tasks for themselves to retain or regain their independent living skills and to build their confidence during/after a period of ill-health. Done effectively, reablement can enhance people’s independence, and reduce their dependency on adult social care. It further helps discharge Individual’s from hospital, prevent readmission, and enable them to remain living at home. This vision and ethos inform the Council objectives and key deliverables. RBC are commissioning a "block contract" that procures the services of two locally based home care providers to deliver 70 hours of Reablement provision per week EACH (thus 140 hours in total shared between two providers). If the Service is used to its maximum capacity EACH Provider will deliver 280 hours of reablement provision on a 4-weekly cycle. This also equates to 3,650 hours a year EACH (based on 52.14 weeks). This could be subject to change during the contract lifecycle depending on the level of demand for services.The reablement provision will be available to Individual’s for a maximum of (6) weeks at no charge to the Individual. The service will follow ‘Pathway 1’ hospital discharges coordinated by the ‘Intermediate Care and Hospital Team’ who will retain the responsibility to oversee the accompanying reviews. The Council does not exclude the possibility of wider avenues of referrals in the future.
