The successful provider will proactively manage and deliver the step-up model in Somerset and will deliver all care and support required, within a residential setting based in Taunton or Yeovil.

two.2.4) Description of the procurement

The Provider will be required:

• To provide and staff premises for the delivery of the contract.

• To develop a collaborative approach between a broad network of clinical, social and community services which all place emphasis on the person’s needs rather than on organisational boundaries.

• To deliver care and support with an emphasis on goal setting, in a collaborative way that optimizes independence and wellbeing.

• To deliver a person-centred, adaptive and flexible intervention tailored to each person.

• To ensure that people are supported on the basis of comprehensive assessment, resulting in a structured individual care plan that involves active therapy, treatment or opportunity for recovery.

• To deliver time limited interventions (an average of 72hours for step-up care) and to support people to make the necessary arrangements to move onto more independent solutions.

• To provide all daily consumables, housekeeping items and personal hygiene products required for the running of the service, i.e. toilet roll, tissues, toothpaste, cleaning products.

• To ensure that the environment is safe for patients and that the premises are secure against unauthorised entry.
