Ensuring the service is well led in-line with CQC domain definitions and fully supported to deliver high quality primary medical level of care and oversight for residents. Patients may have transferred from acute care or require step up from community-based care, are medically fit and require a period of assessment, rehabilitation recovery and/or reablement to achieve discharge into an appropriate community setting. Medical duties
• Support the residents care Mon - Fri 8am – 6pm (after which the out of hours provision will activate) with medical advice, support, education, supervision and clinical care management of the patients within the unit.
• Support the transition of care into community settings, focusing on safe discharge within length of stay ambitions.
• Provide clinical support in the form of ad hoc assessments as required by patient presentation.
• To respond in a timely way to clinical management issues and make senior decisions as required.
• Ensure the patient journey is timely and patients are not unduly delayed in the Lings Bar unit, supporting the transition of care into community settings as appropriate to their needs.
• Provide telephone support as required within a 2-hour response timeframe.
 Reconcile and prescribe medications relating to admissions into the unit as required via the NHCT System One unit.
 Ordering of TTO’s to support timely discharge from the unit back into community.

Service model
 Daily provision of medical advice and support as required within the hours of 8-6 for a minimum of 20 hours per week, excluding bank holidays and to be the senior decision maker on clinical care management of the residents within the service
 Support of the Advanced Care Planning MDT discussion
 Provide 52 weeks medical cover and support for all residents
 Prescribe medication on admission into the facility as required and liaise with internal Pharmacy colleagues on the reconciliation of medication to optimise patient outcomes
 Care records will be managed via an NHCT SystmOne unit and it is expected all medical information will be managed on this system.
