NHS West Yorkshire Integrated Care Board (Bradford District and Craven Health and Care Partnership) is looking to commission the provision of Home Oxygen Service Assessment and Review (HOSAR).
The home oxygen service assesses, reviews and follows up patients who require oxygen at home in a timely manner. The service will cover patients across Bradford Districts and Craven and will review all patients currently receiving oxygen to determine what supply and equipment is appropriate for their condition and will support and follow up those patients. It is vitally important that communications across health and care partners and other statutory organisations are established and maintained to improve outcomes for patients.
