Provision of community and accommodation-based housing related support to improve outcomes for vulnerable people who are homeless or at risk of homelessness.
The Services will provide flexible and responsive support to vulnerable people who are in need of support because they are homeless or at risk of becoming homeless. This may include but not be limited to, people with substance misuse issues (including dependency and co-occurring conditions), mental health problems, physical disabilities or long-term physical health conditions, learning disabilities (where Service Users do not reach the threshold for adult social care) and those with a history of offending behaviours.
The entire geography of Hampshire is not covered in this tender. Separate grant arrangements will be in place with Winchester City Council, New Forest district Council and Basingstoke and Deane Borough Council.
