Warwickshire County Council (the Council) is seeking a single provider, consortia or similar to provide an integrated physical health service whereby Health Professionals, Warwickshire residents and patients can access a range of physical health services to prevent physical ill-health, improve physical health and wellbeing and reduce inequalities.

The service will include the following core elements:

1. Single Point of Access and Universal Support (triage)

2. Physical Activity on Referral and Weight Management

3. Smoking Cessation

4. NHS Health Checks - supporting Primary Care, delivering Community Health Checks and Point of Care Testing

two.2.4) Description of the procurement

Warwickshire County Council (the Council) is seeking a single provider, consortia or similar to provide an integrated physical health service whereby Health Professionals, Warwickshire residents and patients can access a range of physical health services to prevent physical ill-health, improve physical health and wellbeing and reduce inequalities.

The procurement will be an open procedure.

These services may be subject to modifications and changes in order to meet the changing needs of the Council and the service and increases or decreases in funding.

The initial contract term will be for 3 years however the Council reserves the right to extend the contract by further periods not exceeding 24 months (24 being the maximum available extension period) at the discretion of the Council based on the contract performance of the successful applicant and available funding

The Council will undertake its financial assessment of the tendering organisation (and if considered necessary by the Council, any partners within a consortium bid) based on

its own evaluation of the tenderers' most recent 2 years accounts (or if 2 years accounts are not available, equivalent evidence which confirms financial viability).
