The purpose of the service is to support Service Users and their relatives / carers from the start of their hospital stay up to 3 weeks post discharge. This service would support safe and timely discharge when there is no ‘Reason to Reside’ in a hospital bed and the Home First principles. This service will also help to reduce bed shortages within secondary care through improved patient flow.
