two.2.4) Description of the procurement

The TLHC Programme is a flagship national pilot programme funded by NHS England, designed to contribute to the Long-Term Plan ambition of detecting 75% of all cancers at Stage 1 or 2 by 2028.

In localities where the service is live, all people aged between 55 and 74, who have ever smoked, are invited to have a lung health check assessment that assesses the risk of them having a lung cancer. Those deemed at higher risk are invited to receive a low-dose CT scan to identify possible lung cancers.

The national cancer programme has approved Phase 4 expansion of the Cheshire & Merseyside (C&M) Targeted Lung Health Check (TLHC) pilot footprint to Wirral, Warrington, and North Sefton (Southport and Formby) during 2023/24. To achieve NHSE deliverables set out in the 2023/24. Operational Planning Guidance states that these Places must be mobilised by the end of Quarter 4.

The potential contract will be valued at approximately £2,7m* per year; and the contract's duration will be for 4 years.

Explanation:

Liverpool Heart and Chest Hospital NHS Foundation Trust has a rolling contract with the ICB for specialist cardiothoracic and respiratory activity and has delivered Phases 1 to 3 of the TLHC service in a strong partnership with the respective NHS Place-based leads. It has established a proven model that delivers an integrated approach with scanning and radiology, Multi Disciplinary Team (MDT) review and direct referral to lung cancer clinics with infrastructure and interfaces, clinical and operational leadership, data sharing, and governance structures already in place.

There are significant advantages to working with our existing provider in terms of programme continuity, speed of mobilisation, their local knowledge of the area and associated health inequalities, provision of a whole/wrap around service and delivery within the timescales expected by the National Cancer Programme.

The introduction of a new provider could introduce variation in the TLHC offer/pathway across the C&M footprint.

In June 2023, the transition of the pilot to a national screening programme was announced. Guidance as to how the transition to national screening programme will take place and a national delivery model is yet to be published. A direct award enables continuation of the proven model in the interim and avoids the demand on resource and delay that would be inherent to completing a full procurement process.
