Over the last 2 years, NHS England has been testing real-world implementation models for delivering a Total Diet Replacement (TDR) based intervention to people with type 2 diabetes. NHS England has to date commissioned this service in 21 Integrated Care Systems (ICSs) and is now preparing to expand the programme further, to achieve full national coverage in 2023/24.

Purpose of this document
This notice aims to provide an update on plans to carry out a procurement process to invite bids to join a new national framework of Low Calorie Diet Programme providers. Details included here are provided to support potential providers to plan for the bidding period once the Invitation to Tender is published. All details described at this stage remain subject to change – information in the Invitation to Tender will take precedence over the information in this notice.

two.2.4) Description of the procurement

There will be a national procurement for a framework of up to 5 providers with call-off contracts being awarded on an Integrated Care System (ICS) footprint, with a phased roll-out across England in 2023/24.

Providers will be expected to develop their own relationships to ensure that they are able to offer the choice of delivery channels to individuals, as well as products and equipment as required within the service specification (including Total Diet Replacement (TDR) products that comply with all applicable legislative requirements and standards and equipment for monitoring weight, blood pressure and blood glucose), ensuring that the delivery channels are delivered through a single provider organisation, even where more than one organisation is involved such as a Lead Provider and sub-contractor arrangement. These relationships and the processes supporting them, will need to be clearly detailed in the bidding process.

Purpose of this document
This document aims to provide an update on plans to carry out a procurement process to invite bids to join a new national framework of Low Calorie Diet Programme providers. Details included here are provided to support potential providers to plan for the bidding period once the Invitation to Tender is published. All details described at this stage remain subject to change – information in the Invitation to Tender will take precedence over the information in this document.

Delivery channels and providing choice
The currently planned approach will require providers to offer a choice of delivery channels to eligible individuals choosing to join the programme. Individuals will have the right to choose to receive the support digitally or in-person through a one-to-one face to face model.

a. Digital: Contact between the provider and the participant would be via digital technology, through mediums such as online messaging, video calling, telephone conversations or using smart phone app. The digital provision must accommodate different levels of user capability to enable participants of varying levels of competency to access the intervention.

b. One to one (in person): Predominantly in-person one to one support package for the duration of the intervention.

Providers will be required to offer the programme in both formats, with an individual’s choice of delivery model dependent on their preference. There must be a process in place to allow individuals to have a free and open choice of either delivery channel.

Payment model
NHS England plans to adopt a standardised payment profile for all providers, in line with the payment approach used during the pilot period. This aims to provide an incentive to enroll and retain more individuals on programmes.
The following table sets out the planned payment profile:

Milestone 1 (assessment, attendance at first intervention session and commenced TDR) -30%
Milestone 2 (completion of 12 weeks of TDR) - 30%
Milestone 3 (completion of the whole programme) - 30%
Milestone 4 (outcome based – at least 10% weight loss at 12 months) - 10%

Next steps
Subject to internal approvals, it is anticipated that a Contract Notice and the Invitation to Tender will be published in September/October 2022 with call-off contracts awarded pursuant to mini-competitions before the end of May 2023.

NHS England is not inviting questions in response to this document. Once the Invitation to Tender has been published, there will be a period in which bidders are able to submit clarification questions.
