Section four. Procedure

four.1) Description

four.1.1) Type of procedure

Award of a contract without prior publication of a call for competition in the cases listed below

  • The procurement falls outside the scope of application of the regulations

Explanation:

There is no alternative to the CUSA device. There are alternatives which operate in a different manner (e.g. Aquamantis, Harmonic) however none of these allow the precise dissection and ligation of critical liver structures such as biliary pedicles and hepatic veins. In addition, from my experience very few, if any liver surgeons would contemplate carrying out major liver resection without CUSA. The incidence if bile leak post liver resection with CUSA is low (happy to provide my figures) which significantly reduces post-operative morbidity, hospital stay and expenditure (e.g., ERCP). The use of CUSA ensure the safe use of liver surgery for patient at RMH.
