Scope
Description
The commissioning of the Hounslow integrated health and wellbeing service affords an opportunity to learn from the impact of the pandemic and address key health and wellbeing issues that affect our residents and communities. This is a harsh reminder of the persistent unequal burden of ill health, the widening gap of healthy life expectancy experienced in our communities, and the avoidable difference between health and wellbeing that we must address.
The new integrated service aims to generate positive health impact on the lives of our residents; empowering them to achieve their best health and wellbeing, whilst reducing the widening gap of health inequalities.
