East Sussex County Council wishes to commission a service provider to design, deliver and evaluate an Integrated Health and Wellbeing Service.

The service will predominantly support residents of East Sussex to make positive changes to their behavioural risk factors to improve their health and wellbeing and reduce their risk of developing conditions such as diabetes, cancer and heart disease.

The service will include the provision of:

• One-to-one behaviour change support.

• Specialist stop smoking support.

• Tier 2 adult weight management support.

• Community NHS Health Checks.

• Making Every Contact Counts training.
