Somerset NHS Foundation Trust is embarking upon an exciting project to develop community respiratory and frailty services under the auspices of the NHS Hospital@Home initiative, the aim being to reduce the dependence of patients and other agencies upon acute sector beds. Other specialities may also be added to the scope of works during the contract period.

two.2.4) Description of the procurement

The Somerset Virtual Ward will commence shortly. The team will provide support for both early hospital discharge and admissions avoidance, undertake a variety of interventions to support patients, including home assessment, telephone/video contact and the application of digital monitoring systems to manage a caseload. The team will link seamlessly with colleagues in primary care, secondary care, and other developing elements of the Hospital@Home service, so providing virtual ward support for patients who would otherwise be in hospital.

These demonstrations will help the Trust to develop a specification for a future procurement exercise. No contract will be awarded as a result of this PIN.
