(1) Community @home service: provision of dedicated GP, driver and vehicle to support Urgent Community Response provision, in the home setting. Estimated 180 contract clinical hours per week (9,351 hours per year) and 264 contract driver hours per week (13,732 hours per year).

(2) Acute GP hub: provision of x1 GP at St Thomas’ hospital and x2 GPs at Guy’s Urgent Care Centre. Estimated 162 contract clinical hours per week (8,424 hours per year) for Guys UCC and 101 contract clinical hours per week (5,278 per year) for St Thomas' Hospital site.
