The Borough Council of Calderdale (the "Council") invites Tenders from those who wish to be considered for the Living Well with Dementia - Community Support, Advice and Information Service (the "Service"). It is anticipated that the contract will commence 1st April 2022 for a period up to 31st March 2025 with an option to extend for two (2) further periods of twelve (12) months each. The service is to be delivered throughout the Borough of Calderdale.

two.2.4) Description of the procurement

The Borough Council of Calderdale (the "Council") invites Tenders from those who wish to be considered for the Living Well with Dementia - Community Support, Advice and Information Service (the "Service"). It is anticipated that the contract will commence 1st April 2022 for a period up to 31st March 2025 with an option to extend for two (2) further periods of twelve (12) months each. The service is to be delivered throughout the Borough of Calderdale.

The Service will provide timely and responsive support to individuals who have a diagnosis of dementia, their families and carers to help them cope with the changes they need to make to their lives to ensure they are able to cope long-term with the psychological, social and physical effects of the condition. The Service will provide appropriate information, support, advice and signposting to maximise the benefits to the individual their family and Carers from diagnosis to end of life, whether that be living in their own home, in hospital or in long term care.

Quality will equate to 90% of the total evaluation scoring criteria and price will equate to 10%.

We will operate a threshold on the Quality responses, any submissions that achieve below 50% of the quality scores available will be deemed to be non-compliant and will be eliminated from the tender process
