Description
The City of Wolverhampton Council is reviewing the market with a view to re tendering our existing supported living provision.
Tapworks is a City of Wolverhampton Council building that was specifically set up to ensure suitable supported living provision for those with mental health and complex needs. The scheme comprises of 14 one-bedroom flats for adults aged 18 and above with complex mental health needs. Tapworks Supported Living Service supports people with complex mental health needs to individuals who have had chaotic lifestyles and who would benefit from a re-ablement programme delivered within a 24-hour service. They will generally be:
Aged 18 years and over.
Reside in hospital including low/medium secure settings, residential or nursing care provision.
Are at risk of relapse and recall to hospital.
A Wolverhampton resident who is willing to accept care/support from the provider as identified in their care/support plan, with the aim being to maximise independence.
Able to maintain accommodation to an acceptable standard with support as identified via social work assessment.
Have been assessed by social work team as having a need for care and support to enable them to live as independently as possible
Have needs that can be met within the Service
