two.1.4) Short description

To provide specialist assessment of veterans’ social care needs across the South East. This will include provision of social care coordination and intensive support to veterans experiencing social care crisis whereby the individual’s social needs are interfering with their ability to engage with an appropriate mental health pathway. It will require supporting veterans with issues around employment, housing, debt / welfare, disability, social inclusion and prescribing at the earliest opportunity following case allocation when clinically appropriate to do so. This support will be provided to veterans who may lack social support and are more likely to disengage from services or those who are deemed to need an additional level of intensive and face to face support.

Those working on this contract will work with, and be supported by, NHS clinical staff providing veteran’s services as part of the stepped model of care.

This tender is a joint tender for Berkshire Healthcare NHS Foundation Trust and Sussex Partnership NHS Foundation Trust. Each party will award its own contract.

Lot 1 will cover Berkshire, Oxfordshire, Buckinghamshire , Hampshire and Isle of Wight. Lot 2 will cover Kent, Surrey, & Sussex. Bidders must tender for both Lots.
