The Council is looking to commission the Home Support Immediate Response Service. This service provides temporary and short-term care and reablement support that prevents either hospital admissions or the need for long term care packages.

two.2.4) Description of the procurement

The Council is looking to commission the Home Support Immediate Response Service. This service provides temporary and short-term care and reablement support that prevents either hospital admissions or the need for long term care packages.

The service will provide additional, flexible capacity to help people to go home from hospital as soon as they are ready; and to support the Council and NHS partners to smooth temporary fluctuations in the supply and demand of home support services.

The Council will pay for a block payment for home support hours delivered. The block payment will comprise of 100 hours per week but may be extended at the Commissioner’s discretion subject to demand and other relevant factors.

The service will:

- Facilitate timely hospital discharges, providing suitable temporary and short-term care packages.

- Meet the needs of people requiring home support to live independently including short term reablement support.

- Help avoid admission into hospital.

- Help people move to long-term service provider quickly and with minimal disruption to their support plan and daily routines.

The Contract will start on 01/11/2022 and end on 31/10//2023 with options to extend for 2 further periods of 12 months.
