Specialist Palliative Care is provided in the home or other residential setting in the community. Services are available at different times over a 7 day period. Care is delivered to patients in the Community through a core specialist multidisciplinary team, led by a Community Nurse Specialist, to provide hollistic care. The Health Board wish to renew the contract with Marie Curie to continue their Specialist Palliative Care Service within the Vale of Glamorgan.

two.2.4) Description of the procurement

Specialist Palliative Care Services provided within the community. All patients have an agreed care plan and can be referred to other services. Care will include complex symptom control, specialist palliative rehabilitation and coordination of care at the end of life. Palliative care education and advice is provided to other health care professionals. All patients admitted to the specialist community care services are assessed by the medical team/nursing team/ multidisciplinary team. The MDT devise a care plan / goals for the patients care which is agreed by the patient / family. An agreed set of services is put in place. The care given and planned is reviewed on a regular basis by the MDT or appropriate health care professionals through formal review criteria. For patients who require discharge from the service there is formal discharge criteria and agreed pathways for referral to services.

The Contract will be awarded for a 5 year period on a 3 plus 2 year extension option

Explanation:

The Cardiff and Vale University Health Board wish to renew the existing contract with Marie Curie Cancer Care for a 5 Year Period. The Health Board have been in negotiation with Marie Curie to agree the funding level required to support the level of service requirements to meet the demand within the Cardiff and Vale of Glamorgan. The renewal will provide seamless service continuation.
