An eyecare service for Service Users over 18 years of age who are registered with a GP practice located within the geographical area of Norfolk and Waveney Integrated Care Board.
Consisting of:
Community Glaucoma - Level 1 Service
Community Glaucoma - Level 2 Service
Community Post-Operative Cataract Service
Community Glaucoma - Level 1 Service
a: Intraocular Pressure Reading: Applanation Tonometry (by either Goldmann or Perkins Applanation tonometry)
b: Visual Field Check
c: IOP repeat readings service level 1c
Community Glaucoma - Level 2 Service
The service assists Hospital Eye Services (HES) to reduce the number of consultations for service users who are diagnosed as having Ocular Hypertension (OHT) i.e. consistently high intra-ocular pressures (IOP) but no glaucoma, the service provides monitoring for service users with OHT who may be likely to develop glaucoma. This monitoring service allows service users with diagnosed OHT and no treatment to be discharged back into primary care for monitoring by community optometrists.
The service is provided by accredited local ophthalmic practitioners who have a range of equipment
to facilitate a detailed examination of the eye.
Service users are referred to this service on discharge from HES.
Service users are monitored for 5 years by the service and may be referred back to the HES if necessary.
Community Post-Operative Cataract Service
Service is provided by accredited local ophthalmic practitioners who have a range of equipment to facilitate detailed examination of the eye, as well as specialist knowledge and skill to counsel and advise a patient post-surgery.
This service provides assessment and management of patients who have undergone cataract surgery in either or both eyes.
Patients without complications post-surgery will be instructed to visit the referring optometrist after 4 weeks for the final post-operative examination and GOS examination.
Cataract post-op assessment includes:
• Review progress and medication
• Dilation of pupil for patients with an unexpected visual acuity of 6/12 or worse
• Slit lamp examination of the anterior eye and posterior eye
• Discussion of the patient's satisfaction with the outcome of surgery
• Record of vision and determination of prescription required for any residual refractive error
• Discuss second eye surgery where appropriate
• Collect and submit outcome data
