two.2.4) Description of the procurement
NHS Norfolk and Waveney Clinical Commissioning Group are seeking a suitably qualified and capable Provider/s to provide a dementia service across Norfolk and Waveney.
The Specialist Integrated Dementia Support Service (SIDSS) will provide a comprehensive integrated specialist support service for people living with dementia in Norfolk & Waveney. It will include clinical and non-clinical support and offer specialist advice, information and support to people living with dementia, their families and carers. This will include support through the diagnostic pathway.
The service will offer families affected by dementia individualised, accessible and proactive support that enables them to live as well as possible with the condition. This will improve the outcomes and experiences of health and social care services for this population. This will be achieved by delivering high quality, efficient service closer to home, ensuring that those affected get the right care and support at the right time.
