Description
The Council is seeking providers interested in delivering the Community Reablement at Home Pilot service in Hackney. The service will provide Reablement support for people in their homes will be delivered by commissioned provider(s), with a focus on enabling individuals to achieve therapist-set goals over a period of up to 6 weeks.

Description of how multiple lots may be awarded:
Lot 1: Up to 4 suppliers will be awarded, based on their weighting of both price and quality.
Lot 2: Highest overall scoring supplier on both price and quality will be awarded.
Both lots have a minimum threshold of 50% for Quality scores - any bidders scoring under this will be excluded automatically.
In the event that any suppliers response to a quality question scores a zero to any answer the supplier will be immediately excluded.
In the event that any supplier scores two or more ‘one’s’ in their quality questions, they will be immediately excluded.
