two.1.4) Short description

Lambeth’s home care service is being re-commissioned under a new neighbourhood model that complements the way that health, social care and wellbeing services in Lambeth are going to be organised around neighbourhoods. The neighbourhood model of care will allow for greater integration, continuity of care and use of community assets, to the benefit of home care service users. The proposed procurement of home care services will mean that in each of the eight neighbourhoods there will be one home care provider that will deliver all standard home care packages to residents in that neighbourhood, which we estimate to be 90-95% of care to be delivered. In addition there will be a Reserved Provider List for provision of services/care packages where a Neighbourhood provider cannot accept all referrals in their neighbourhood area to provide opportunity SME's to bid. The Contract will be for 5 years with the option to extend for a further 2 years.
