Cystinosis is a very rare inherited lysosomal storage disease presenting in infancy (95% of cases) with severe kidney disease but subsequently involving multiple organs (eyes, thyroid, liver, diabetes, muscle deterioration). Patients often present to paediatric nephrology services although multi organ disease requires multi-speciality input. Effective drug therapy and renal transplantation now allows survival into the fifth decade.

This project comprises a market engagement exercise to ascertain the competition and capacity within the market including potential collaboration opportunities amongst providers. Following this, where there is a single provider expressing an interest in a Lot who would be able to meet the requirements, direct awards may be made. Where there are several providers within a lot, a further ‘light-touch’ one stage provider selection approach will be required.

NHS Arden and Greater East Midlands Commissioning Support Unit (AGCSU), on behalf of NHS England (referred to as the Commissioner), is seeking to establish Cystinosis Diagnosis and Management Advice Services and are inviting interested providers to respond to this market engagement exercise. The proposed service model is described in the NHS England service specification for the provision of Cystinosis diagnosis and co-ordination of management (all ages). This sets out the detailed objectives and clinical criteria for the services.

The prevalence of Cystinosis in Europe is 0.5 per 100,000. There are approximately 200-300 patients in the UK, this is based on prescription records and reports from the patients’ society Cystinosis Foundation UK.

NHS England is looking to gauge the potential market interest for providing these services and would like to hear from suitably qualified and experienced providers with the necessary capacity and capability (or a demonstrable ability to acquire the necessary capacity and capability) within the market including potential collaboration opportunities amongst providers. Following this, where there is a single provider expressing an interest in a Lot who would be able to meet the requirements, direct awards may be made. Where there are several providers within a lot, a further ‘light-touch’ one stage provider selection approach will be required.

Partnerships/integrated working will be required between adults and paediatric services; in particular to support the transition pathway for patients.

In line with the service specification, services will be provided from 3 adult and 3 paediatric hubs. These will be selected to offer geographically equitable access to minimise patient travel times whilst also offering access to multidisciplinary resources and to support national networking of the highly specialised centres with the adult and paediatric centres working closely together to offer an integrated service and excellent transition of patients. The hubs will be required to work in a national network to support the development of national policies and protocols and to facilitate research.

The centre allocation model will be based on a regional lot breakdown as follows:
