Hawkhurst Community Hospital provides a 22-bed inpatient ward for patients requiring (pathway 2) short term rehabilitation and recovery. Patients are routinely admitted from either from an acute hospital or from the community. The current service provision delivers a streamlined clinical pathway from Community Hospital to post discharge / home for local residents

Integration, Collaboration and Service Sustainability; The current service provision delivers a streamlined clinical pathway from Community Hospital to post discharge / home for local residents. Whilst the model doesn't align with other Community Hospital medical models, it currently delivers operational resilience, which in its absence in the short term would cause challenges regarding sustainability

Improving Access, reducing Health Inequalities and facilitating choice and Social Value; Delivery through a local primary care provider increases the likelihood of understanding population health and community social integration

Value; following negotiations between both Parties, KCHFT is satisfied with the service and value for money.

Quality and Innovation; Weald View Medical Practice provide a sufficient medical requirement in the wards to support initial clinical assessment, support for prescribing, assessment and intervention for clinical deterioration or not thriving, and discharge planning to meet the needs of the patient cohort to a satisfactory standard
