two.2.4) Description of the procurement
To provide a dedicated Care Home EOLC resource to support care home staff and residents with assessments, advance care planning and appropriate treatment plans.
1. Improve access to specialist EOLC support for residents
• Proactively contacting care homes to seek out new referrals
or provide advice and support with existing care plans on a
weekly basis.
• Promote a clear and safe referral pathway for EOLC referrals supporting residents with urgent and routine EOLC needs.
• Promote clear route for OOHs support for EOLC issues
2. Support Care home MDT around advance care plans with
specialist EOLC advice.
• Attend (in person or virtually) care home MDT meetings to
provide expert advice to the wider team around residents
EOLC plans.
• Establish strong relationships with the wider care home team
including nursing staff, care staff, GPs, other care home in
reach staff and LAS.
• Promote use of CMC/UCP as a shared record of advance care
plans
3. Support the development of EOLC knowledge and skills
within care homes.
• Identify gaps in knowledge and awareness of EOLC within the
care homes and address this work proactively with care homes
to make assessment of needs around EOLC, working with the
staff, other parts of the care home in reach services and the
wider MDT to address any gaps in knowledge / awareness.
• Provide appropriate training opportunities for care home team
whether one to one support or group training / awareness
sessions.
• Provide advice on system changes within the care home that
will improve health and wellbeing outcomes and experience
for residents and their families at the end phase of their lives.
This is a Provider Selection Regime (PSR) confirmation of contract award notice. This contract has been awarded under the Health Care Services (Provider Selection regime) Regulations 2023.
For the avoidance of doubt, the provisions of the Public Contracts Regulations 2015 do not apply to this award. BHCIC has formally awarded this contract to the Royal Trinity Hospice for three years with a 12 month notice of termination from 1/4/2024 to 31/3 2027using Direct Award Process A. The lifetime value of the contract is £212,162.13.
