The aim of the DPS is to allow for the creation of more capacity and choice for Older People and their families, whilst allowing a range of service providers to work together in partnership(s) to ensure that services provided in the Extra Care Housing schemes are seamless and outcomes which cross specific individual services can be measured and improved.
