Bedford Borough Council (‘the Authority’) is inviting Tenders from organisations wishing to provide Home from Hospital Service and Home Support and Reablement Service and Supporting Independence Service. These Services are Better Care Fund (BCF) funded and are advertised under two Lots:• Lot 1: Home from Hospital Service and Home Support and Reablement Service• Lot 2: Supporting Independence ServiceProviders are invited to bid for both or either of the Lots as detailed above. The Authority is intended to issue a separate contract, substantially on the same terms for each Lot.

two.2.4) Description of the procurement

The Home from Hospital Service and Home Support and Reablement Service includes two separate elements of delivery. The first is home from hospital and the second is home support and reablement Service. Both elements include conveying people home from hospital who have been discharged (also those relayed to hospital, who have not been admitted). Clients may require the intervention of either or both elements of this Service to prevent their re-admission, ensuring that their home environment is safe and appropriate for them to return to.

The Supporting Independence Service provides support to individuals affected by long term physical health conditions, physical inactivity and social isolation to help them gain confidence, self-manage and reconnect to their community, therefore regain and maintain their independence. The Service is provided to Bedford Borough residents up to a period of time determined by their needs but no longer than 12 weeks.
