St Helens Borough Council Public Health Team are looking to commission a borough wide multi-disciplinary Falls Prevention Service. The service will meet NICE Guidance providing appropriate assessment, identification of falls risk / reasons for fall, and then tailored intervention to manage risk. Assessments and interventions will be provided at the individual’s convenience in their own home, care home or local community setting and will adopt a multi-agency, multi-disciplinary, multi-factorial approach with evidence-based interventions and evaluation. The Falls Prevention Service will work closely with the providers of strength and balance activities and coordinate the local Falls Prevention Partnership.
