The service will be visible and community facing with a proactive focus on promoting early identification, providing a personalised response, and signposting people to appropriate support within their local neighbourhoods. This will include talking with people to help them to understand and manage the impact dementia has on their lives and make informed choices in planning for their future. The service will work to bring together, simplify, and coordinate timely and appropriate high-quality information, advice, and support, ensuring this is paced according to individual needs and requirements. Support will also be provided to carers and the wider family to improve their health and emotional wellbeing, and help equip them with the resilience, tools and practical strategies required for their caring role.
