Manchester City Council Public Health Department is looking to enter into a contract for a social prescribing service in Manchester. The service provides social prescribing interventions for people in Manchester who have one or more long term conditions and/or whose social circumstances place them at risk of developing long-term conditions or becoming frequent users of health and care services in the future. Its focus is on connecting people to community-based sources of social and non-medical support, which includes time-limited one-to-one support where required.
Manchester City Council is intending to award the contract to an existing provider following Direct Award Process C under the Health Care Services (Provider Selection Regime) Regulations 2023.
The contract is for three years running from April 2025 to March 2028.
