The notice is in accordance with The Health Care Services (Provider Selection Regime) Regulations 2023, No. 1348 - PART 2 - Regulation 13, Modification of contracts and framework agreements during their term and Schedule 12 Notice of contract or framework agreement modification.
The change in lifetime value of the contract as a result of this modification, including all previous modifications is 6.6% of the original value.
There is no change to the length of the agreement.
Details of decision makers are a Consultant in Public Health, Principal Public Health Specialist, Director of Public Health and Cabinet Member for Adults and Wellbeing.
There are no declared or potential conflicts of interest (COIs) of individuals making the decision.
The delay in publication of this notice is due to the technical issue of submitting a modification notice without a relevant, original notice number. As this arrangement was conducted under Reg.12, no such notice existed. NHS England provided the notice number highlighted above to allow us to publish this modification notice.

seven.2.1) Description of the modifications
Nature and extent of the modifications (with indication of possible earlier changes to the contract):
This modification will continue to support improvements to the delivery of the HCP through increased reach to children, young people and families and will also enable more support to the frontline workforce. Identified key success measures include:
Continuous improvement and/or maintenance of KPIs.
Targeted antenatal contact.
Exception reporting.
Additional Healthy Child contacts as per schedule of interventions.
Contribute to the Speech and Language support pathway in the early years (WELCOMM tool).
Increased Breastfeeding rates.
Support the SEND (Special Educational Needs and Disabilities) agenda.
Expectant and new parents' resilience support.
Increased universal provision for perinatal mental health and post-natal depression.
Development of the School Nurse service offer to support the recommendations of the Emotional Health Needs Assessment, relationships and sex education and sexual health provision.

Need for modification brought about by circumstances which a diligent contracting authority/entity could not foresee.

the modification is made in response to external factors beyond the control of the Council and the provider including, changes in patient or service user volume, the modification does not render the contract materially different in character.
The partnership agreement is an integral part of Wolverhampton's health delivery and there is no economic benefit to the Council or service delivery benefit to the service user in changing to a different provider, and would cause duplication of cost and avoidable resourcing implications.
