two.2.4) Description of the procurement

Slough Borough Council is seeking an experienced provider for the delivery of an Integrated Health and Wellbeing Service for residents of Slough. The core purpose of the service is to improve the health and wellbeing outcomes of Slough residents by implementing an Integrated Health and Wellbeing Lifestyle Service, including health education and outreach, behaviour change assessments, alcohol brief advice, adult weight management, falls prevention smoking cessation and youth vaping cessation, community health checks and CVD risk prevention, and liaison services with partners for referral generation .The Contract start date is the 1st April 2025 and will be for a term of five years valued at £700,000 per annum with a total budget of £3,500,000 over five years. It will be a 3 year contract with a possible two one year extensions of one year.
