NHS West Yorkshire Integrated Care Board (WY ICB) is inviting interested bidders to apply to be accredited for the AQP Consultant-Led Community Ophthalmology Services.
Please note that the service is to be delivered to Leeds patients only and not across West Yorkshire.
The WY ICB is re-opening this window for new entrants to join the Any Qualified Provider (AQP) list. We are also requesting that current providers re-apply by completing the required sections to ensure that they still comply with the requirements of the qualification criteria.
The aims of the service are to provide a comprehensive consultant-led community ophthalmology services and to provide a full local pathway for Leeds patients encompassing:
- Clinical assessment and triage
- Opinion and Advice for patients and referrers
- Treatment and recovery where applicable
- Discharge back to the appropriate level of care
- Referral onwards only where appropriate
The service will also:
- Offer patients a choice of locations and appointment times, ensuring equitable, timely, and appropriate access.
- Ensure that patients are seen and treated in a safe environment most appropriate to their care needs.
Provide best value for money, delivering effective and sustainable use of resources.
