The NHS Wales Shared Services Partnership, hosted by Velindre NHS Trust are acting on behalf of Cardiff and Vale University Local Health Board who are issuing this PIN to engage with the market to understand options available for a digital system to provide access to share clinical pathways from the community (including non health partners) through to the hospital settings.

two.2.4) Description of the procurement
The Health Board is undertaking significant service transformational work and requires a flexible and adaptable resource to contain localised clinical pathways. The resource is required to accommodate rapid changes and updates to pathways and to accommodate whole systems pathways from the community (including non health partners) through to the hospital setting. It should also be able to accommodate patient/person facing information and be easily accessible to a wide range of health clinicians, professional practitioners and partners as each pathway specifies. The resource must be highly acceptable to practitioners with demonstrable level of active engagement and utilisation and have an ability to benchmark, access and learn from other communities utilising pathways resources.
The pathways are expected to contain medical guidance on the assessment and management of clinical conditions and administrative information on access to diagnostics, specialist opinion, specialist treatment services and other supports including the collaboration of all organisations implementing this system allowing each organisation to share knowledge, processes and pathways to improve patient treatments and experiences.
