Nature and extent of the modifications (with indication of possible earlier changes to the contract):
This modification is to show that the NHS NET CUF uplift of 2.83% has been applied to the price per treatment for both In-Centre and Home Haemodialysis clinical provision. This will apply from 03.12.2025 until 02.12.2026. This will change the lifetime of the contract value if the contract was extended to its maximum term of £31,052,936. The effective date of modification is 03.12.2025
