NHS South-East London Integrated Care Board (ICB) - Bromley has a statutory duty to meet the requirements of the (i) National Framework for Children and Young People's Continuing Care (CC) and (ii) National Framework for NHS Continuing Healthcare (CHC) and NHS-Funded Nursing Care. Overall the two frameworks constitute the key elements of All-Age Continuing Care (AACC).
NHS South-East London Integrated Care Board (ICB) - Bromley has established an in-house AACC service comprising of managers, clinical leads, nurses and support officers. The service has a particular duty to deliver the ICB's statutory requirements and to manage the AACC budget. This service specification is for an All Age Continuing Care (AACC) Partnership Delivery Service (PDS) which will work alongside the ICB in-house AACC team to deliver the totality of requirements for the service.
The PDS will be a clinical-led service that will deliver critical CHC nurse-led assessments and reviews, in support of the completion of Multi-Disciplinary Team (MDT) Decision Support Tools (DSTs). The service will also provide additional AACC administrative support services.

NHS South-East London Integrated Care Board (ICB) - Bromley has a statutory duty to meet the requirements of the (i) National Framework for Children and Young People's Continuing Care (CC) and (ii) National Framework for NHS Continuing Healthcare (CHC) and NHS-Funded Nursing Care. Overall the two frameworks constitute the key elements of All-Age Continuing Care (AACC).
All Age Continuing Care (AACC) is the provision of community support and care services to eligible children, young people and adults whose primary care need is due to their health condition as opposed to their social care needs. This may include individuals with long-term conditions, who have a disability, or who have an illness, or who have had an accident. For those individuals who are assessed as eligible for AACC, the NHS will fund services to meet their assessed needs. This often includes: domiciliary homecare, a stay at a residential care or nursing home, a respite placement, a personal assistant (PA) or a personal health budget (PHB).
AACC is a critical service for some of the most vulnerable people in society. The services put in place by AACC enable many people with complex needs and health conditions to live healthily at home, to have more independence, to see their friends and families, to access employment, to live without discomfort or to have to worry about their care.

There is an opportunity with the establishment of the AACC PDS service to:
• improve the alignment of ICB and other nursing functions, with improvements around a common approach to nurse quality, training and continuous professional development.
• enhance the overall quality of AACC deliverables in Bromley through a common approach with a partner organisation, drawing expertise and leadership from different areas and providers to improve overall outcomes for AACC eligible clients.
• establish a more sustainable AACC service in Bromley with delivery shared across the in-house team and a partner organisation.
The two elements of the service (ICB and PDS) will need to work in an integrated manner with a single duty function, referral pathway, IT systems and the co-location of staffing.
