Surrey is intending to procuring a new stroke support service which will be delivered for 2 years with an option to extend for a further third year.

The service offers tailored support after discharge from hospital back to an individual’s home or into their community for the short, medium and long term. Commissioners work closely with colleagues from across the health system to ensure that this service is part of the wider Integrated Stroke Delivery Network (ISDN).
