*To increase the level of support to care homes around identification of and support to manage therapy needs in order to further enhance health in care homes.
Establish dedicated Care Home AHP resources (Physiotherapist, Speech and Language Therapist or Occupational Therapist) to support assessments and activate treatment plans more effectively and efficiently within the care home home resident cohort. 4 BHCIC intend to award a contract to an existing provider following Direct Award Process C for three years with a 12 month notice of termination from 1/4/2024 to 31/3 2027. The lifetime value of the contract is £819006.
There will be 2 clear aspects to this work:
1) To look at and address environmental and system factors, such as the knowledge of care home staff, that impact on the health outcomes of residents in relation to AHP therapy needs such as mobility and balance, falls, occupational activity, swallowing, communication etc. The team will work proactively with care homes to in reach to the home and make assessment of needs around therapy provision, working with the staff and other parts of the care home in reach services to address any gaps in knowledge / awareness and to provide advice on environmental / system changes that will improve health and wellbeing outcomes for residents.
2) To provide urgent AHP therapy support for residents and/ or onward signpost into existing community / primary / secondary care services as required
*To provide effective and efficient nutritional management for people of all ages referred with clinical conditions, to improve outcome, promote health and prevent further complications in care homes. To provide dietary assessment, advice and follow-up within the in care homes, providing targeted information relevant to the individual needs of the patient/carer.
