Kent County Council wishes to procure a digital solutions and/or ICT system to facilitating delivery of the NHS Health Check programme. Health Checks are a mandated service that local authorities must provide for their populations, and it has the primary objective of screening for undiagnosed health issues and sending patients along the relevant care pathway to improve outcomes.

Kent County Council is interested in solutions and/or systems that can provide any of, but not limited to, the following functions:

• Identify the eligible population for an NHS Health Check

• Invite eligible patients to an NHS Health Check

• Facilitate the NHS Health Check itself including mandatory data collection during the check

• Report results back to the commissioning local authority

• Allow for detailed analysis of invites, checks, and subsequent outcomes of the eligible population receiving a check

• Manage onward referrals of patients after their check, if required

• Assist with payments of providers e.g., General Practices that may be providing checks

• Provide all digital services in line with strict data security standards and legislation

two.2.4) Description of the procurement

The Council is procuring a system to facilitate the operations, reporting and data management for the Kent NHS Health Check programme. This programme screens 40–74-year-olds without a pre-existing health condition to identify and treat health problems earlier, or to serve as a preventive measure for those that may be at risk of developing health problems.

In Kent, the NHS Health Check programme has been provided by The Council’s public health partner Kent Community Health NHS Foundation Trust (KCHFT) since 2013. KCHFT manage the programme and sub-contract delivery out to approximately 145 General Practices (GPs) and 37 community pharmacies. In addition, KCHFT has an NHS Health Check core team which also provides checks in community clinics. Finally, KCHFT also have an outreach arm which provides NHS Health Checks within various community locations to underserved and underrepresented populations.

A system is required to support the provision of the NHS Health Check programme, with the following being key required functions:

• Regular identification of the eligible population and making these population lists available to Health Check providers

• The means for Health Check providers to invite patients from the eligible population list to an NHS Health Check. As a minimum, SMS and letters must be available.

• The ability to target invitations towards different demographics and/or risk factors e.g., age, ethnicity, at high risk of diabetes etc.

• The ability to exclude non-eligible patients from the eligible population list and any health check data

• A template/module, or other means, for enabling Health Check providers to record a predefined list of medical information during each check

• A means for calculating a QRISK score for each patient following a check, and Health Check providers being able to provide these results to their patients in a variety of formats e.g., printing them out, sending them via secure email.

• Detailed reporting on invites, checks conducted, and subsequent patient outcomes following a check.
