• Work with Primary Care to facilitate access to primary care services such as annual health checks, routine and specialised appointments within Wandsworth
• Enable reasonable adjustments to be made in the processes by which learning disabled patients interact with primary health care services within Wandsworth.
• Enable a co-design approach so that information is developed with the participation of people with learning disabilities.
• Ensure equal access to and communication of primary health care information and services
• Enable People with learning disabilities will have a better understanding of their individual health care and how to manage it.
BHCIC intend to award this contract to an existing provider following Provider Selection regime Direct Award C process. This is a 3-year contract with a 12 month termination notice period and a total contract value of £150000. Service to be provided from 1/4/2024 to 31/3/2027
