Description
The scheme provides temporary accommodation for assessment and rehabilitation for patients leaving hospital, thereby supporting stable housing arrangements and improved flow from hospital and reducing hospital readmission rates. The service consists of four flats borough-wide, offering up to six weeks' accommodation for those unable to return home directly from hospital.
This supports also offers comprehensive long-term needs assessment and helps address issues such as housing, debt, and health.
We are looking for a Provider to deliver this service.

Participation
Legal and financial capacity conditions of participation
The information provided in response to Part 1 - Confirmation of Core Supplier Information will be used by the Council to assess the Economic & Financial Standing of Bidders. A Bidder that is unable to provide trading accounts for the previous 2 years shall provide evidence to support that it has bank accounts and shall provide a balance sheet and income statement to enable the Council to access its Economic & Financial Standing. In addition, the Council will obtain a Company Watch Credit Report.
1. The Company Watch Credit Report, where available, together with the information provided by the Bidder in response to Part 1 - Confirmation of Core Supplier Information, will be used by the Council to undertake detailed financial due diligence. Where the Council concludes that there is insufficient information to assess the Bidder's Economic & Financial Standing, or the information leads to the Council to conclude that the information provided is not acceptable, the Bidder will be excluded.
