The programme has identified 15 pilot sites to test a number of different models of care to better join up services for CYP across all health settings, social care and education.
We are seeking a support partner to work with the national CYP Transformation Programme and the 15 pilot sites for the duration of the programme (approx. 2 years) to develop a national evaluation of the pilot, identify key themes and lessons learned and draft national guidance for use by systems to set out the key principles and themes which underpin excellent integrated care.
