SLAM is seeking to purchase a Caseload Weighting Management Tool / Application with algorithm for predictive analysis functionality (functional or in development) which would enable frontline staff, care coordinators and care teams, plan and prioritise care and resources at a local level based on needs and risk of the patient.

The tool should be able to enable:

• Support, through supervision to ensure flow through the Community pathway.

• Provide drill down facilities from the dashboard view

• Provide Patient Access level data i.e. understanding what the key triggers are and other issues surrounding the patient

• Outward referrals capabilities internal and external

• RAG patients based on needs and risk
