The community MSK Service will provide the following services:
• single point of triage (administrative and clinical);
• assessment (direct and non-direct);
• onward referral as appropriate to secondary care (orthopaedics/rheumatology/pain);
• direct access to diagnostics including X-ray, musculoskeletal ultrasound, musculoskeletal MRI, routine pathology, nerve conduction studies, CT and DEXA scanning;
• disease management and treatment including;
- MSK physiotherapy (exercise, mobilisation, manipulation, splinting, taping, electrotherapy and education informed and supported by research evidence, hydrotherapy where available);
- joint injection therapy;
- pain management;
- Provision of appliances/orthotics/splints.

- Referrer support, training and advice in the management of MSK conditions in primary care

four.1) Description
four.1.1) Form of procedure
Award of a contract without prior publication of a call for competition in the cases listed below

Explanation:
Along with 7 other CCGs in the area, NHS Central London Clinical Commissioning Group is to be incorporated into a new CCG from 1st April 2021 - North West London Clinical Commissioning Group (CCG). A period of review is required from April 2021 to agree the future model of care and to identify the commissioning strategies for services across NW London for the coming year and beyond.
The existing contract is therefore being modified to extend its duration for an interim period of 6 months in order to enable this review to take place
This is an existing service that has been in place since November 2015. The CCG has assessed that the service needs to continue whilst a review is completed, and new commissioning plans are developed by the newly formed North West London CCG from April 2021.
This contract modification is assessed to be proportionate and lawful under Regulation 72(1)(b)
The existing contract is being modified through contract variation to extend its term by 6 months which will increase the overall value of the contract by £1.4m during this additional term.
four.1.11) Main features of the award procedure
The existing contract is therefore being modified to extend its duration for an interim period of 6 months in order to enable this review to take place.
This modification is necessary to ensure continuity of these MSK services for local patients.
The CCG has considered its obligations under the NHS (Procurement Patient Choice and Competition) (No.2) Regulations 2013 and assesses that the incumbent provider offers the safest solution to ensure this continuity at this time, particularly having regard to the priorities and resource pressures associated with the local NHS response to COVID-19.
This contract modification is assessed to be proportionate and lawful under Regulation 72(1)(b)
