Provision of 20 hours of care/support to the step-up/step-down bed in Orchid House Extra Care facility in North Yorkshire
The beds provide a period of rehabilitation for up to 6 weeks for adults over the age of 18, and as a preferred place of care/death for patients in receipt of palliative or end of life care.
The service will:
• Provide a step-up facility for people that cannot remain at home but do not require acute care, admission avoidance.
• Provide a step-down facility for people who have been medically optimised within an acute setting but are not able to return home and require additional support/rehabilitation before returning.
• Provide care and support for people at the end of life / palliative.
• Identifies the patient's key worker who will lead on the planning and discharge
• Promotes individual independence, self-care and well-being.
• Ensure that Carers will participate and follow evidence-based treatment programs to these service users to maximise their recover and independence.

Approximate lifetime value of the contract is £57,200.
Contract term - 1 April 2025 - 31 March 2026 (the contract term may be extended by 1 year).
