The Whole System Integrated Care/Integrated Care Partnership (WSIC/ICP) service in Brent provides a multi-layer level of investment across Primary Care to deliver an integrated model of working between professional, patient and carers in the management of long term conditions. This model puts GP-patient relationship at the centre and used the GP register as a source of the population group.

The service being contracted included risk stratification and case finding, proactive care planning, multi-disciplinary case management and team working, support to self care and complex patient management groups. This service enables integration across the existing primary Care landscape and adds value to what current GP practices can deliver both to patients and to the wider NHS System.

two.2.14) Additional information

An existing contract which has been in place since November 2018 is being modified to effect a 6 month extension to allow the future model of care to be considered by commissioners.

The existing contract value has been £1,013,198.87 until its expiry 31st March 2021. The modification to continue for an additional 6 months will increase this by £228,967.48 until the new expiry of 30th September 2021

Explanation:

A period of review is required from April 2021 to agree the future model of care and to identify the commissioning strategies for urgent care services across NW London for the coming year and beyond.

The existing contract is therefore being modified to extend its duration for an interim period of 6 months in order to enable this review to take place.

This modification is necessary to ensure continuity of these critical services for local patients.

The CCG has considered its obligations under the NHS (Procurement Patient Choice and Competition) (No.2) Regulations 2013 and assesses that the incumbent provider offers the safest solution to ensure this continuity at this time, particularly having regard to the priorities and resource pressures associated with the local NHS response to COVID-19.

This contract modification is assessed to be proportionate and lawful under Regulation 72(1)(b)

Section six. Complementary information

six.3) Additional information

In line with the NWL single CCG commissioning strategy the aim is to standardise the development of a number of emerging Integrated Care Partnerships (ICPs). The extension period of this existing contract will allow Brent Borough to agree a consistent and standardise Brent-wide delivery model to achieve efficiencies in service delivery and avoid duplication within the system.

The 6-month contract extension will allow the CCG to continue the discussions for the smooth transition on the following areas:

• Contractual framework for commissioning at PCN.

• Finance and Staffing Resources at PCN level - determine and agree how resources will be allocated at a PCN level

• Models of Care - agree a consistent and standardise Brent-wide delivery model to achieve efficiencies in service delivery and avoid duplication within the system.

• Reporting - agree a reporting process which takes into account governances and assurances processes.

• Key Performance Indicators (KPIs) - review the current KPIs to see if they are still fit for purpose and are capturing the right information needed for performance reporting and measuring outcomes impacting on patient care.
